On January 15, Shenyang Emergency Management Bureau released the investigation report on the explosion accident of 5 deaths and 52 injuries caused by "October 21" in Shenyang. The cause of the accident was that during the gas grid connection construction of the store, the constructors did not effectively seal after opening the inlet flange, resulting in the leakage of gas through the flange for 3 hours and 25 minutes, The electric spark generated when the mechanical temperature controller of the indoor electric ice display cabinet on the second floor is closed or disconnected will explode, and the explosion equivalent is about 80kg TNT.
On October 21, 2021, Shengwang Erniu barbecue shop, No. 222, Taiyuan South Street, Heping District, Shenyang City, had a pipeline gas leakage explosion accident, resulting in 5 deaths, 3 serious injuries and 49 minor injuries, with a direct economic loss of about 44.25 million yuan. On January 15, Shenyang Emergency Management Bureau released the investigation report of the incident. After investigation, it was determined that the "10.21" large pipeline gas leakage and explosion accident at No. 222, Taiyuan South Street, Heping District, Shenyang was a major production safety responsibility accident.
The report mentioned that the direct cause of the accident was that during the gas grid connection construction of Shengwang Erniu barbecue shop, the construction personnel opened the inlet flange of the inlet pipe valve, and after completing the grid connection construction welding operation, the flange port was not effectively sealed, and the tightness inspection of the flange port was not carried out after ventilation, resulting in the leakage of gas through the flange port, which lasted for 3 hours and 25 minutes.
The leaked gas diffuses naturally in the indoor third floor space of Shengwang Erniu barbecue shop, and the concentration after mixing with the air reaches the explosion limit range (the volume concentration of gas in the first floor space is about 7% - 9%, the second floor space is about 7.2% - 8.5%, and the third floor space is about 7.4% - 8.7%). It explodes when it meets the electric spark generated when the mechanical thermostat of the indoor second floor electric ice display cabinet is closed or disconnected, The explosion equivalent is about 80kg TNT.
According to the requirements of 3.0.9 of gjj51-2016 safety technical specification for operation, maintenance and emergency repair of urban gas facilities: before entering the gas surge chamber, compressor room, metering room, bottle group gasification room, valve chamber, valve well and inspection well, personnel shall first check whether there is gas leakage in the site; Before entering the underground surge chamber, valve well and inspection well for operation, personnel shall also check the concentration of other harmful gases and oxygen, and can enter only after confirming safety. Special personnel shall be assigned to monitor the operation and rotate the operation.
At the same time, 6.1.2 requirements: production operations such as gas shutdown, depressurization, hot work and ventilation of gas facilities shall be equipped with corresponding work tools, communication equipment, protective appliances, fire-fighting equipment, testing instruments, etc.
